HSA  LUNCH  PROGRAM COUPON BOOKS 

The HSA is pleased to announce the opportunity to purchase coupon books for our pizza and hot dog days.  You no longer have to worry about having cash on hand, although cash will still be accepted. 

The coupons are printed in books of ten with a cost of $15.00.  Each coupon is valid for one slice of pizza OR one hot dog.  If your child plans to purchase two slices or two hot dogs, they need to bring either two coupons or cash in the amount of $3.00.  Each coupon is worth $1.50.

PLEASE INFORM YOUR CHILDREN THAT THEY MUST CONTINUE TO RAISE THEIR HAND FOR THE ORDER TAKEN BY THE TEACHER ON PIZZA AND HOT DOG DAYS.  There is no pre-ordering and the count we receive from the classes on these mornings is what determines the pizza and hot dog order.

For safety purposes, please write the name/grade of your child on the back of each coupon so in case it is misplaced at school it can be returned.  Lost coupons cannot be replaced.

Please fill in the form below and return to Marie Grieco, c/o Ryan 4-W.  Your coupons will be returned through the backpack unless otherwise indicated on your order.

Checks should be made payable to OLMIS HSA.  This document will be available on the HSA website, ourladyofmercyhsa.com, in case you need to order additional books down the road.

Please keep in mind when ordering that we have 20 days of pizza remaining in the school year and 5 hot dog days.
